Manually swapped D-pad as Left Axis, and vice-versa, in **main** config window.
Didn't use "Axis to D-pad".
Pressing Up + Left causes buttons not to register properly for a tiny period.

Win7, x64, latest official x360ce x64, Dark Souls 3 x64 - this happened in whole Dark Souls series, didn't check other games. Tested with multiple gamepads.
